A note from our District Nurse...IDPH has recently updated and published the Certificate of Child Health Examination, commonly referred to as the physical exam form.
This form is required for students entering PreK, kindergarten, 6th grade, 9th grade, or starting school in Illinois for the first time. Athletes can use the sports physical form, except for students in 6th grade, 9th grade, or those new to an Illinois school. If you have questions, please contact Nurse K at 217-382-4116.
